Featured is also a risk rating graphic table and a quantitative impact section - records costs to business based on loss duration. 

The form may be of utility when compliance with ISO 22302 is required. It can also help with the creation of a business continuity policy and creating arrangements for potential disruptions. 

The form may also assist with creating a SWOT analysis or vice versa.
